You have a right to access the information recorded about you. The notes do not however, belong to you (they are the property of the NHS ). We are happy to help patients where they wish to exercise this right. We would ask that you write to the Practice Manager with this request. We will find you somewhere comfortable in the practice to look through them. Unfortunately, you cannot take them away from the practice. You can request a photocopy of some or all of you records. There is a charge for this service to cover copying and staff time.
If others, such as solicitors, wish to access you records, a signed consent form must accompany their request from yourself (signed within the preceding 28 days)
