You may register with this practice if you are:
- A student on a course of a minimum of 6 months at Nottingham University, and you live within our catchment area (see map below*)
- A member of staff with a contract of a minimum of 6 months, and you live within our catchment area *
- Spouses and children of students and staff , as above
Most patients will register with us at the beginning of the academic year during a specially organised week, which is usually held in the Trent Building. At other times patients will have to come to the Health Centre to register. If you have your NHS medical card please bring this with you to registration. If you do not have a NHS medical card we will supply you with another form to complete. All new patients over 16 are offered an appointment with our nurses for a health check
This is a group practice and you may see any of the doctors. You have a right to express a preference to see a particular doctor, in which case we will record this and try to ensure, subject to availability of appointments with that particular doctor, that your preference is met.
If you do not meet the above criteria and need immediate care you can still be seen at the health centre as temporary patient. This is only in emergency/urgent situations. If you are not eligible for NHS care (ie not from a country with a reciprocal agreement and not fitting above criteria) then you can be seen as a private patient; there is a charge of £25 for a 10-minute consultation. Under such circumstances all prescriptions are private. There is also a further charge at the pharmacist for any drug prescribed.
You can print the forms required at the following link and then bring them with you:
