Welcome to the Cripps Health Centre Pre-registration page. We understand how much you have to do at the moment with registering for your new university and planning to move away from home (for most of you). When away looking after you health becomes even more important. Most UK students will be registered with a UK General Practitioner/”Family Doctor”. However, it is thought best to register close to where you spend most of the year, which will be your university address. That requires you to register with a G.P. that takes patients from your new locality. We are only one of many practices locally that you could use but many students do choose to register with us. We are in a building on the main campus and so access is easy. We have an active dialogue and relationship with the University so often are involved in supporting students with medical problems and liasing with the University. Our list size is approximately 27,000, so you can see we serve much of the Nottingham University community. We have 9 dedicated doctors, 5 nurses and all the associated staff you would expect, including Health Visitor, Midwife, visiting psychiatrist and physiotherapist (provided by the university – clinic within the health centre).

To be eligible for our practice we ask that you are on a course or employed by the Nottingham University (or spouse/child of someone eligible) and fall within our practice boundaries (almost all students will as the boundaries are set to areas most students live).

Persons from abroad, studying fulltime for a minimum of 6 months may also register (for periods shorter that 6 months you must be seen as a private patient and each consultation is chargeable). To make the registration process as straight forward as possible we ask you to print the next 3 pages, complete and bring them with you to the health centre. Returning students who have either not registered at Cripps previously or registered elsewhere over the summer/last year will need to contact the health centre.

Please fill in all information as accurately as you can as it may form the basis of care. Where possible include doses and frequency of medication. Be particularly careful with allergy information and immunization history as incorrect information can put you at risk. Your current GP should be able to provide this information but be patient they may have a number of requests and this is not urgent (allow time for your current practice to deal with it).

You are to bring the following forms with when registering with the health centre. If you do not have them you will have to complete them again from memory and at worst may not provide us with information that affects your medical care.
