Patient’s consent form for video/audio-recording of consultations.

  • We are hoping to make video/audio recordings of some of the consultations between patients and the Doctor whom you are seeing today.
  • The video/audio is ONLY of you and the doctor talking together. All recordings are carried out according to guidelines issued by the General Medical Council and will be stored securely in line with the General Data Protection Regulation (GDPR). They will be deleted by 26 weeks (182 days) of the recording taking place. Intimate examinations will not be recorded, and the camera/recorder will be switched off on request
  • The video/audio will be heard only by doctors involved in the assessment of doctors as part of their training for General Practice. The tape will be stored securely and treated as confidentially as any other medical record. The tape will be erased as soon as practicable but before 26 weeks.
  • The Doctor you consult with is responsible for the security and confidentiality of the video/audio recording. If the tape/recording is to leave the premises it will sent by registered post or under an encrypted service.
  • You do not have to agree to your consultation with the doctor being recorded. If you want the video/audio turned off, please tell Reception – this is not a problem, and will not affect your consultation or the care you receive in any way.
  • Even if you give permission now, you may change your mind later. You may ask for the video/audio to be switched off at any time. If, after the consultation, you would prefer that the tape is not used, please contact the receptionist who will ensure that your consultation is erased from the tape.
  • But if you do not mind your consultation being recorded, we are grateful to you. Improving the assessment of GPs should lead to a better service to patients.
  • If you consent to this consultation being video/audio recorded, please respond to the accurx message sent (either a YES or a NO). In addition, the doctor consulting with you will confirm verbally before the consultation proceeds. You can change your mind at any stage.

  • After you have finished speaking with the doctor, if you would prefer the tape is not used, please contact the receptionist who will inform the doctor to ensure it is erased.
