New Patient Registration

Welcome to Nottingham.

Most UK students will be registered with a UK General Practitioner/Family Doctor. However, NHS guidance strongly advises patients to register with a practice close to where you spend most of the year, which will be your university address.

The University of Nottingham Health Service is an NHS General Practice with a strong working relationship in supporting the health needs of the University community.

You may register with the University of Nottingham Health Service (UNHS) if you are a:

• student on a course of a minimum of three months at The University of Nottingham living within our catchment area (see map opposite)
• member of staff on a contract of at least six months and you live within our catchment area
• spouse, child or dependent of students and staff (as above)

Online Registration 

To register with the practice you will need your Nottingham address.  Please ensure you have this before proceeding.

This for new registrations only. To change your address please contact the practice directly.

Please ensure that all fields in the registration forms are completed.  Although it may be tempting to skip a few and hope for the best they are all important and without the full details we cannot process your registration.  If there are any queries we will contact you by phone, email or text message.  We have a limited window in which to edit your registration and resubmit so please do respond asap.

IMPORTANT - Nottingham is your new home town.  The 'home address' section on the registration form refers to your new Nottingham address.  If you are a student currently living elsewhere and moving to Nottingham please put your current address in the previous address section.

What happens next

Please allow 7 days for your registration to be processed, we will send you a text message to confirm when you are registered. 

If  you are currently registered with a GP in England or Wales your medical records should transfer electronically.  However if you are registered outside of England and Wales we will not automatically receive your medical history unless you provide a copy.  (Records from Scotland are transferred but there is often a delay of up to 12 months )

Allocation of Named Accountable GP

All patients who register with us, including children, are allocated a named, accountable GP who is responsible for their overall care at the practice.  This is referred to as your 'Usual GP'.  You should be informed of your Usual GP when you register but you can check who this is with any member of staff at any time.  We will always make reasonable efforts to accommodate requests to change the Usual GP.  This does not change your ability to request to see any of our individual GP's at any stage.

Important vaccinations

Meningitis

If you are under 25 and entering higher education for the first time, you should receive the MenACWY vaccine, ideally before coming to the University. If you have not already been immunised before arrival, this can be done at Cripps Health Centre

The MenACWY vaccine is given by a single injection into the upper arm and protects against 4 strains of the meningococcal bacteria – A, C, W and Y – which cause meningitis and blood poisoning (septicaemia).You can find further information about meningitis symptoms on the NHS Website https://www.nhs.uk/conditions/meningitis/symptoms/

Measles, Mumps and Rubella

Measles and mumps are circulating in the general UK population, particularly among young people.  Several outbreaks of mumps have occurred at both universities in Nottingham and, year-on-year, the number of cases continues to be significant. If you are not certain that you have received the two MMR immunisations, then in line with national policy, we recommend that you obtain them before arriving at University with a month between each dose.

Tetanus, diphtheria and polio

You should make sure your vaccinations for these are up to date. In the UK you may have received them as the 3-in-one teenage booster.

Human papillomavirus (HPV)

HPV is the name of a very common group of viruses. They do not cause any problems in most people, but some types can cause genital warts or cancer.  If you are under 25 and have not received this vaccination we recommend that you do.

Covid-19

Covid-19 vaccinations are the best way of protecting yourself and others against becoming seriously ill or having long-term effects. We would encourage and recommend all students to get this vaccination and necessary boosters prior to arrival at University.

Anyone in the UK aged 5 or over can now get vaccinated. Please follow this link for bookings:  NHS online vaccination booking service.

If you’re outside the UK, and have access to a UK-approved vaccine, please get vaccinated as soon as you can. If you don’t have access, we expect that you will be able to get vaccinated for free when you arrive in the UK, regardless of your nationality or immigration

Summary Care Records

When you register with a GP in England, the practice creates something called a Summary Care Record (SCR).

The SCR will include information about you such as your name, address, date of birth and NHS number. It will also contain details of any current medication, allergies or bad reactions you may have had to medicines in the past.

Having an SCR means that anyone involved in your care has access to this vital information. For example, if you need urgent treatment at a hospital or other NHS location, staff will know everything they need to about your health to give you the best possible treatment.

Not having a Summary Care Record means crucial decisions about your treatment may be delayed or made without the complete information.

However, you have the choice to opt out of having an SCR. You will need to complete an SCR patient consent preference form. and return it to your GP practice.

National Opt-Out

If you do not want your data to be shared for purposes other than your healthcare, you need to complete a National Opt-Out.

Data sharing is vital to making sure that the NHS understands and adapts to patients’ changing needs. Data is also crucial in developing new initiatives, services and treatments such as vaccines. The NHS’s response to the COVID-19 pandemic, particularly the development and distribution of vaccines, would not have been possible without access to such vital information.

However, you have the choice to opt out of having your data used for planning and research purposes.
